Items to bring to your appointment:

• Previous year's tax return
• All W-2'sSocial Security 1099
• Pension statements Social Security Card
• If you are filing as disabled, the initial letter that states the date you were classified as disabled
• BANK STATEMENTS CANNOT BE ACCEPTED​ 

Get help applying for your Medicare card.

• Enroll in or renew your Medicare Part D (prescription drug plan).
• Must be 64 years and 9 months old (3 months before 65th birthday) OR disabled if under 64 years old.
• Appointment required, call 217-732-6132 to schedule. Appointments are typically scheduled on Monday mornings (but exceptions can be made, if needed). 

Items to bring to your appointment:

• Medicare card (red, white, and blue)
• All insurance cards (supplemental insurance and/or prescription drug cards)
• All prescription drugs in their bottles OR a pharmacy printout of all current prescriptions

• Learn about what is covered under Medicare, Medicare Supplements and Medicare Advantage Plans, and Medicare Part D.
• Learn how to save and not overpay for Medicare insurance & how you can be penalized under Medicare.
